I was at the range today with a friend who has a fairly new S&W M&P15.  It would fire one or two rounds and then nothing.  Manual cycling the action would sometimes result in a dead trigger as well.  Removing the magazine and dry firing would work most of the time but occasionally it would not fire.  Seems like the trigger was not being reset.  It's only about 6 months old so he'll be contacting S&W about sending it in.  I was just wondering if any one has had this happen or had a guess as to the cause.

Sounds like a problem with the disconnector or the disconnector spring?

I had a similiar problem with my AR. What was happening was when the trigger would reset it was binding. It was locking back too far down. My buddy looked at the gun for me and showed me. He gave me a new hammer and spring and the problem was solved. People here diagnosed my disconnector spring and it wasn't the issue. I'd check the spring first and if not move onto the hammer.

Take the upper off and have a look in there. Could be a primer hung up in the works. Make sure when you release the hammer you don't let it strike the lower,(stick yer finger in between where the hammer wants to hit.

i suggest this but dont stick your finger where the hammer hits that seems like it would hurt, i would grab the hammer and guide it to rest
