Armalite M4 upper with standard handguards, had about 1k thru it (mostly wolf, fed xm193, win Q3131A1) with no functional problems whatsoever. I noticed when mounting a newly purchased EOTech 512 that the center rib of the hanguard is canted to the left slightly. Barrel nut does not appear to be binding the gas tube, though the gap between the barrel nut teeth and the tube isn't perfectly even on both sides (it is touching or damn near touching the gas tube on one side). Should I align the barrel nut better or just leave it alone?  As stated, so far gun has been running fine.

Kinda what I was thinkin', just thought I'd ask.  Maybe see if it's a common issue or not.  Was also concered as I was contemplating putting a DD Omega rail on it and was concerned with whether or not the alignment would be off.  I lack the tools and knowledge to mess with it myself.

I agree.

I'm still a bit curious as to whether this would lead to a railed handguard being canted as well.  I have my eye on either a DD Omega or a KAC RAS.  I lack the tools and ability to adjust the barrel nut.

Will it cause problems with a rail up front? Probably. Brother had a S&W 15x with the Troy drop in handrail. Exact same problem you described. Barrel nut was overtightened so his rail was canted. It may depend on the rail you plan to use, but the Troy indexes itself into the barrel nut grooves, so if the nut is too far one way or another the rail ends up being off.
