I'm not sure if this is all of the factors but they are the ones that come immediatly to mind:

Breathing
Heart beat
Inconsistant upward pressure on the barrel from resting the barrel or non-free floated handguard.
Inconsistant pressure on the buttstock usually under the toe of the buttstock.
Improper torque on the barrel nut.
Inconsistant head position on the buttstock fore to aft (cheek weld).

Solutions:

Breathing- Try making the trigger break at the same point in your breathing cycle.
Heart beat- Try to slow heart rate down and get trigger to break at the same point in pulse.
Inconsistant upward pressure on the barrel from resting the barrel or non-free floated handguard.  Free float the barrel and do not rest the barrel on anything while shooting don't use barrel mounted crap, especially slings or bipods that mount to the barrel vice a freefloated handguard
Inconsistant pressure on the buttstock usually under the toe of the buttstock.  Use of a small beanbag under the toe of the butstock or no pressure on the toe of the butstock.
Improper torque on the barrel nut.  Gunsmith, or if you have the tools, check it
Inconsistant head position on the buttstock fore to aft (cheek weld).  This is why several military folks are trained to put their nose on the charging handle, to provide a consistant position of the head in relation to the sights.[/blue]

Also, removeable sights or scopes/mounts may be working loose.  Improper paralax from a scope could also be a contributing factor,  as could inconsistant powder charge in ammo.

That's all I can think of right now.

Good advice so far. What really affect my rifle is when I shoot with a bipod directly off the bench. It seems the barrel "bounces" a little differently each time. Solution: put something with a little give under th bipod e.g. folded towel, foam, gun case. The difference is pretty dramatic. About 3-4" sometimes.

Are you using a scope or irons? What type of target are you shooting at?

Build-up of barrel heat.
