Is it really worth the extra money, how much accuracy is really gained?

unscientific answer, I don't think so, I freefloated an AR after shooting it for a couple months and changed nothing else i.e. ammo,sights,cheekweld,range,and distance:  and my accuracy remained exactly the same. It looks cool though!

Are you asking about picatinny FF's?

They can help if you're hanging a ton of stuff past the receiver...

With the new breed of FF's that don't req. full barrel disassembly, I think they are worth it.

That said, I've got one of each...both perform quite well!

I think it's less about accuracy, and more about stability of the platform.  If you are talking about rails, as GhostRing says, hangin' stuff on wobbly handguards feels terrible compared to FF.
