I just purchased the 18 in SS Keymod complete upper.  While waiting for the blemish lower I was able to seat the bolt and pull charging handle no issues.  Today the lower showed up put them together no issues.  I then took it out to test fire.  Loaded the PMAG inserted and mag would not lock into place, pulled bolt back mag seated.  I let the bolt go forward and it did not seat fully, pulled handle and would not release.  So bolt won't go fully closed, have a live round in the chamber.  I was able to get the lower off and looked from the bottom.  The bolt teeth are not lined up with the barrel so when you pull back on the charging handle they catch on the barrel.  

Any ideas on how to fix this?

Edit: guess I can take the rail, and barrel off to get the round out and then figure out why it happened.
2nd edit...had some sleep, pulling rail and barrel will not work, the lugs on the bolt are "caught" in the lugs on the barrel...going out of town for the weekend, be back late Sunday I will attempt to get photo's of the issue and update if I have any success.

What you have described is a common problem caused by ammo that was not manufactured correctly or was damaged before it was chambered.  Many rifles will not easily lock a magazine with the bolt forward, and the bolt is supposed to lock into the barrel extension.  That is how the rifle works.

The common solution is to use a rubber mallet to tap the charging handle and get the round out.  Try some different rounds from the same batch.  If it still does it, try another brand of ammo.  Unless the problem shows up with every round you try to chamber, across two or three different types of ammo, sending the rifle in for customer service is a waste of time.

So I believe I have found the cause, I was able to get the Bolt out.  It appears the extractor spring is to tight.  I tried with multiple brands of ammo with the same results.  Once I disassembled the bolt and reassembled the problem went from every round to every 2nd to 3rd round.  This was the same across multiple brands of ammo including my own reloads.  As another test I loaded all rounds that had the issue into another gun/same mag no issues. Again I believe the problem is in the extractor.

Update, new bolt did not fix the issue.  PSA could you guys respond to my email about sending the entire upper back?  I was asked to provide info in an email last Friday, which I did and still have not heard anything back as well as sending a follow up email yesterday about this.

Have you since received your label? CS states they just got to your email today. You should have received an email from the CS team, as well as an email direct from UPS with your return label.

Have you tried taking one of the O-rings out from the extractor? Two O-rings is a COMMON problem with 308 bolts and I wish manufacturers would stop doing it. It's not necessary to have two, but they keep jamming them in there anyway.

Yep tried that, I thought it was a bit of over kill.  

I did receive the label.  Upper is on its way back now.  Hoping this gets resolved, really looking forward to shooting it.

So got the upper back with no indication of what was repaired however I can clearly say nothing was as the very first round into the upper did the exact same thing bolt fails to extract.  Very frustrated, bought this in mid may and now mid July and still have not been able to shoot it.

Edit: finally got time to take a closer look, removed the second o-ring and was able to cycle a complete mag.  I am going to take it out to shoot tomorrow and will update from there.

I have no idea why manufacturers still think the 308 needs two O-rings behind the extractor. It does NOT. This is the #1 cause of issues with 308. PSA (and others)... stop using double O-rings for christs sake.

So got out this evening and Shot/tested the rifle.  The bugs are gone.  Ran 100 rounds through it, 20 suppressed.  No issues and once it was zeroed it was amazingly accurate.  I am running a Sig Scope on it
