How do you make a PDF like this that you can fill in on the screen?
What program, anyone know?

In Acrobat, it's Tools > Advanced Editing > Forms > Text Field.



If you're importing a Word document, there might be some way to sort of automate the conversion. I've seen it done (by one of my secretaries) but haven't done it myself.

That form is fill enabled.  If you want to know how to make one of a different form, it's easy with full version Acrobat 9.  The form wizard will auto create the fields and the user just needs to tweak it a bit.  If you have something you want done, shoot it to me and I can create it in a matter of minutes.

Open Office (openoffice.org) will let you create pdf's.

google for cute pdf. it installs like a printer and its free. anything you can print, you can make into a pdf file.
