How do you make a PDF like this that you can fill in on the screen?

That form is fill enabled. If you want to know how to make one of a different form, it's easy with full version Acrobat 9. The form wizard will auto create the fields and the user just needs to tweak it a bit. If you have something you want done, shoot it to me and I can create it in a matter of minutes. |
