Around here all the firefighters and EMT's wear headphones while driving.
This applies to the firetrucks (enclosed cab) and E350 based aid-cars.
Why?

I know some have commo in them so they can all communicate with each other. with the sirens on sometimes you cant hear crap. but I am not a firefighter. I was a medic we had radios

Over time the noise of the siren + motor makes us deaf. NFPA recommends those headsets so you can talk to each other in the rig AND so you don't go deaf. (As fast)

So we can talk and hear each other / radio in the cab. Especially for the #3 and #4 guys (who ride in the back) so they know whats going on.
