Click on "Profile" then "Service" then "Forms".

You need to download the following forms:
AR15-SF223 - Request for Voluntary Separation
AR15-SF308 - Affidavit of Condition of Mental Health
AR15-SF556 - Affidavit of Acceptance of Separation Policies
AR15-SF762 - Affidavit of Consent to Future Mockery

Fill each one out, in triplicate, and send to ARFcom HQ, c/o Dept. DISENGAGE!!!

I filed mine out in January 2002. Should be hearing back from the Goat soon.
