It all depends on who is granting the security clearance, and what the level that you are trying to
attain. The higher the level, the deeper they dig. Criminal history, family background (ex. - having
an uncle who is ex-KGB won't sit well), financial history (bankruptcies, etc.), etc. are all looked at.
Last time I heard, clearances are taking abt 1 - 1 1/2 years to complete new ones. The backlog is
ridiculous.

DOE (Dept of Energy) has their standard, then DOD & DIA have their own also, I believe.

Just answer all questions truthfully and completely.  Dig up old residence addresses back at least 15 years - this requirement was dropped for DOD clearances for a while, but I talked to folks last week that had to enter this info on recent paperwork.  You should also dig up names and addresses of employers back 10 years or so.

I had to get Top Secret Clearance (ooooh!!) for working on the Psych ward at Ft. Sam Houston back in '85 or '86.

I got cleared an none of my immediate family got calls, probably just background checks is all I can guess unless things have changed which I'm sure they have.

The form your looking for is called the SF-86, and can be found on many mil.gov websites for download.

You can print it out and fill it out and sign it and mail it to whoever requested that you do a security clearance test.

I have the PDF file on my computer already because I had to fill out the form when I applied at the Puget Sound Naval Station.

It is fairly in depth, long(13 or 14 pages) but isn't too terribly hard to fill out.

Biggest thing is that you be honest on the form.  If you tried mekong mind fuck 12 years ago, don't lie about it, just admit it.  Its not what you have done necessarily, its whether you lie about it or not that concerns them.  Unless you were busted for felonies such as rape, murder, robbery etc. Then it does matter.

HERE is a PDF online version that I found. I don't know if this is the newest or most updated one, so it would be best to check for a newer version elsewhere.

Anymore, they have to have about as much evidence as it would take to indict you if they want to deny you a security clearance. Not like the old days...
