[ARCHIVED THREAD] - Another C&R question

I downloaded the form 7CR from Cruffler. I filled it out. I made a 2copies. I wrote the letter to my CLEO, and attached one of the copies to it. Other than writing a check. Are there any other form(s) to fill out? I don't want to mail this stuff in and then look like an idiot.
