I'm trying to figure out how this happened. The previous bullet was perfect. I had to take the die apart and tap out that little piece that came off. It fits the bullet piece stuck in the brass perfectly, like a hat.

How did it separate?
How did it not get seated in the brass to a normal depth?
Why is the chunk in the brass rounded in the middle?

Poorly cast.  It happens.

I normally use plated bullets, so this is a first for me.

Cold mold. I've made a few of those in my casting days.

I have done my fair share of reloading but no expert , are you belling the case mouth enough ? I noticed with cast lead bullets if the case mouth is not belled enough lead will shear off of previous rounds and build up in the die causing a lead build up on subsequent rounds.

If you've loaded quite a few, it's probably lead shavings that have built up in your seating die and came off on that last loaded cartridge. You might need to bell your casemouths a little more.

ETA, Beat by a minute!

That's what i thought initially. I checked OAL and it's consistent up until this guy.

Been there done that, and this would be my suspicion as well.

I'd say cold joint in the casting

I just started checking OAL on all of them and it goes up and down like a rollercoaster. I must of hit the highs when i was checking periodically. Well, time to pull some bullets. Thanks guys.

As to why the little lead slug is rounded in the middle, that's because of the shape of the nose punch on the seating die.

Flat nose punches are usually available, although at added expense.

I thought the bullet had separated but now I'm sure it's build up of shavings in the die. So it's the original tip.

The smallest OAL is 1.4" (38 special). I'm just going to shoot it out of a ruger blackhawk, that chunky bastard don't care.

If the bullet is too far out of the case, that could be a problem. If you are using a certain power it might cause a high pressure problem. Depends on the amount of empty air space inside the case.

This. 38spcl is notorious for that.

EDIT: the reason is because 38spcl cases can vary wildly in oal, and when you set your bell with one, it might be too much or too little for the next case.

All the folks talking about cast bullet issues... I don't think so, if he is using the Hornady bullets pictured... those are swaged lead. Not cast.

Yup - I'm betting that is a swaged bullet, not cast.

Just like the good old Winchester 148 Hollow base wad cutter, swaged, then lube grooves are knurled into the sides.
