I'm at the range and I had something happen that I've never seen before. I was shooting my AR, fired a round, pulled the trigger on the next round and nothing happened. I checked and the bolt had not fully closed. I ejected the round and I saw inbetween the locking lugs and the chamber that there was a chunk of metal. I shook it out and it was a mangled primer. I checked the spent rounds on the ground and I found one with the primer missing. This was a factory round, Winchester I believe (old mag I loaded a long time ago). I shot the rest of my ammo fine.

It could be a lot of things.  If that's all you have to worry about you're doing fine.  Keep shooting.

I've had it happen three or four times.  Twice, I didn't even know it happened until the primers fell out of my lower when I removed the upper for cleaning afterwards.
