Very unfortunate story about the 7 young marines in training accident.
Mortar training accident story

My question is what makes the 81mm so different that prevents double loading vs. the 60mm as the story suggest?

Actually in either one, a round can get stuck while dropping down the tube.  They trained us (Armor/Cav Office Basic Course 1977) to watch for the round to come out of the tube (firing) before hanging the next round.  If it did not come out/fire, the immediate action drill was to kick the tube.  If it still did not fire, it was a PITA to remove the tube from the baseplate and elevate the base so the round would slide back out of the muzzle.  The man on the muzzle end cupped his hands to catch the shoulders of the round and not hit the fuze.
