Can arfcom add some color to my thoughts?

The other day, I was involved in a MVA at work. I was driving a vehicle lift at work. When driving in a gate area, my brakes (foot brake and hand brake) went out, and in turn, I struck a baggage tug that was being driven by someone else. Thankfully there were no injuries, and only minor damage to the vehicles. However, I've been going over the whole incident in my mind several times over the past few days, and I still keep getting a sick feeling in my stomach. I have the idea that had I not started yelling when I realized that I had no brakes and couldn't stop, this person probably would have dismounted off of the tug and have been in the direct path of where my vehicle would have ended, definitely severely injuring, and possibly killing them. Thankfully that hasn't happened, but I'm not getting much peace out of this.

I'd have similar problems under your shoes. Take solace in the fact that nobody died.

I was involved as a member of the flight crew in a plane crash due to multiple mechanical failures.  No one was seriously injured, but the plane was destroyed.  Even though I wasn't captain, I still felt responsible for what happened, despite knowing 100% that both he and I did absolutely everything correctly to prevent, assess, and respond to the situation.  I used to lie awake at night trying to think of any way that I could have prevented what happened, or successfully saved the airplane, even though in the end it's about the people, and the metal is always a secondary concern.

Sometimes you just have to make peace that there was nothing you could have done better, and that you did the best that anyone could do in that situation.  If mechanical failure is truly the cause, no one can second-guess your actions while standing on firm ground.

This.  Move on and quit dwelling all about yourself.  Company's got insurance for this kind of thing after all.
