It's one of the included pieces in the Aimpoint M68 CCO packaging. It's used to mount an aimpoint Comp M2 on a fixed carry handle while still retaining full use of the irons.

Looks very similar to a cheap carry handle mount someone gave me with some equally cheap red dot.  Was designed to mount the optic forward of the carry handle to co-witness the irons.  Problem was the damn thing was so poorly made that it couldn't be tightened enough to keep it from rocking around like a teenage boy talking to the Milf next door.  I tossed both in a box and haven't seen them since.
