So say you are selling a gun privately to a person in another state. What if the ffl he sends you is missing the top header of the form, is signed but has no printed name, position or date? What if, also, you are sending this through an ffl? Is this copy sufficient for the sending ffl's records?

Exactly.

Just use the info you have to check that it's a valid FFL.  If it is, no problem.

Yeah I understand the ffl check. But I'm asking about shipping a gun from ffl to ffl.

If you are the sending FFL then the receiving FFL needs to send you a full copy of his license.  If you're not you shouldn't get anything except the first 3 and last 5 digits of his license number so you can use EZCheck.
