Failed inspection yesterday because the left back brake is not aligned.

What could cause this? And how does it effect driving?

Is this an easy job to do for a mechanic? Or pretty time consuming?

What make,  model,  year?  What is the problem?

Does it need a brake "alignment" or brake lining?  Lining is the friction material that is bonded to the brake shoe.  If it is worn out it will need replaced and the drums turned to true them up.  You will probably need wheel cylinders and springs too.

A brake adjustment will move the shoe and lining closer to the drum.  This will give you a higher or firmer pedal.  If one side is not self-adjusting itself it could cause a pull.
The vehicle will need jacked up and the brake drums removed.  Then inspect everything.  If it greasy and smells like gear oil you will need a new axle seal.  If it is wet with brake fluid it will need wheel cylinders.
