Has anyone ever actually taken it?

There have been a lot of threads on this. In fact, FishMox and other fish med threads seem to pop-up almost daily, so if you search this forum and its archive you'll find more info than you probably want.

That said, several people have used it. Nobody that I know of has reported any problem.

Those who report taking it generally report a positive result, BUT keep in mind that lots of bugs clear-up all by themselves without treatment, so anecdotal evidence on antibiotic efficacy is pretty worthless. If you come down with a head cold and take colloidal silver/gorillacillin/tincture of unicorn bunghole/hot tea prayed over by tibetan monks/nothing at all, you will be better in 7 days, and not on account of whatever it is you took.

If you decide to keep antibiotics of any type on-hand, be sure to understand how and when to use them. Go to your doc when you can and don't self-treat when there are other options available. YMMV.

I searched, and couldn't find where someone has actually taken it. I'm mainly concerned ity if its safe for humans.

That's weird. I would have sworn there was a guy within the last week who responded to one of these threads with a positive comment about having taken fish meds, but now I can't find that post.

My best guess at an answer to your question is 'probably.' That's not very helpful, though, when you're trying to prepare yourself. Here's an alternative that has worked for me:

I travel a lot, so I ask my doc to write me prescriptions for simple, useful travel meds off of Walmart's $4 script list–– phenergan (for nausea), doxycycline (my choice of antimalarial), ciprofloxacin, cephalexin, and cotrimoxazole. I also get metronidazole, but I see it isn't on the list. I have a legitimate use for the meds when I travel and my doc doesn't mind writing for travel meds. I never ask for anything questionable like narcotics.

Here's a PDF of Walmart's $4 drug list.

It's the same stuff, made in the same factory, in the same capsules, in the same dosages, in the same bottle with a different label as the human stuff.

I've heard that before. However, some that I have seen came packaged in non-dissolving plastic capsules. That leads me to suspect that there may be some difference at least with some manufacturers. These were two different brands purchased several years ago.
