I read the " Everything you wanted to know about suppresors" but I can't find anything about the correct ATF form I need.
I went to the ATF web site and there is nothing about suppresors but I did find forms regarding building firearms.
Can someone point me in the right direction and maybe a link.

This is my first attempt at NFA stuff.

A suppresor is a firearm to them.

4.b. for Type of Firearm should be filled out as silencer.

Do not use TitleII's Form 1, it is outdated.
Use this one: Cyclic's Version
You can even click on the picture block and import a jpg file.

supressor forms should be done by your dealer unless you are "making" it yourself...you pay the dealer a transfer fee, let them earn it :)

This, my dealer had all my forms filled out for me.
I taped on the pictures and headed to my CLEO.

I am in the early stages of an MK12 mod 0 build. Looking at the difference in price between buying a complete upper from ADCO or building it myself. I haven't bought anything yet.

I hear the wait time for approval from the ATF can take four to six months. In my infanent wisdom I thought I would let that time take place while I'm doing the build. Am I putting the cart before to horse by applying for the suppressor this early? If I decide to build it myself would ADCO(I plan on getting the can from them) still do the paperwork or would they ask for a copy at time of purchase if I already have the stamp?

The form 1 is to build the supressor yourself.  You fill it out, send it in, get the stamp and then build it yourself.

If someone else builds it and you buy it from a dealer, then the dealer fills out a form 4, you sign and attach your picture and mail it to the ATF and wait for the stamp to get sent to the dealer.  Then you go to the dealer and pick it up.
