[ARCHIVED THREAD] - Form Request/Help?

I need to transfer some class 3 items from one of my LLC's to another LLC. Does anyone have a link or copy of a Form 4. And don't I just fill it out and pay the tax and send it in? The dealer has always done them but that was because they were coming from him to me and I believe this would be a straightforward thing I should just do myself. Thanks
