In box 4H do I put what I will be having engraved on the rifle or every thing that is already engraved on the rifle? (model number, made in where ever, caliber, etc.)

You don't need to list the original manufacturer info, as that is already going to be in block 4(a).
In block 4(h), you can put what you will have engraved on the rifle (name, city, state), or you can leave it blank. ATF will approve it either way.

DO NOT enter anything in 4h...
Recent/past topics seem to have created trouble as far as entering stuff in 4h. Leave it blank.
