10-inch or 12-inch.  I have the 18" version for slicing cakes, and it's like a freaking laser beam, it's so sharp that it just slices through effortlessly and cleanly, at least on cakes.    I'm sure it wouldn't feel like a laser beam going through a hard crust, but this knife does cut bread and cake more easily and cleanly than any bread knife I've ever tried.  They also make offset versions if that floats your boat.

As to a guide, IDK.  The only guides I use for slicing are for cakes, I just cut bread loaves by eye.

My wife bought me this set when she bought me a bread machine back in the day, shown here with one of the first loaves made with my new KitchenAid mixer. The knife is the standard serrated bread knife, but the whole thing folds up for storage in a bag, and the knife snaps on the far side magnetically.

I bought a cheap meat slicer on amazon. Works pretty good on bread, and I can slice meat.
