Do they have to issue a firearms permit to qualified individuals, or is it along the lines of CLEO signatures for NFA items (in free states) where the Chief can sign or not sign depending on how he felt when he woke up that morning, and the individual has no recourse?

A friend asked, I don't personally know anyone who was denied, so I can't say for sure.

Are you are talking about registering approved long arms with the Dept of Revenue.

As far as the "law" is concerned:

Specifically 8-20-060, 8-20-080, 8-20-100, 8-20-110

8-20-110 provides:

(a) Upon receipt of a properly executed application for a registration certificate and the report of the superintendent [re: whether the info on the application checks out], the mayor, upon determining that the applicant has complied with the provisions of this chapter, shall authorize the issuance of the registration certificate. [SNIP]

(b) The mayor shall approve or deny any application for a registration certificate within a 120-day period beginning on the date the superintendent receives the application unless good cause is shown. In the case of an application to reregister a firearm currently registered, the mayor shall have 365 days after receipt of such application to approve or deny such application unless good cause is shown.

If you can get yourself tied to a PI, security company, or bale bondsman, you may be able to get a Tan Card. (CCW to and from work only) Nothing is available for us regular folk though.

Do you know the statute that forbids the Chicago Police from accepting aplications for handguns?
Is it an ordnance or an "internal decision"?

In 1982, Mayor Jane Byrne and the city council began to hold hearings on an ordinance proposed by alderman Ed Burke banning the further sale and registration of handguns in Chicago. Receiving strong support from Byrne and her allies, and coming in the wake of the assassination attempts on President Reagan and Pope John Paul II, the ordinance passed. All residents who purchased and registered their handguns prior to January 1982 were allowed to keep their weapons. Chicago became the first major city to enact a handgun freeze in United States history.

Yes, it's an ordinance. It's very simple, actually. And it is all right HERE if you read through it all.

8-20-040 says: "All firearms in the City of Chicago shall be registered . . . . No person shall within the City of Chicago, possess, harbor, have under his control, transfer, offer for sale, sell, give, deliver, or accept any firearm unless such person is the holder of a valid registration certificate for such firearm. * * *"

8-20-050 (Unregistrable Firearms) says:
"No registration certificate shall be issued for any of the following types of firearms:
[SNIP]
(c) Handguns, except:
(1) Those validly registered to a current owner in the City of Chicago prior to the effective date of this chapter [sometime in 1982], and which contain each of the following: [Ed: trigger locks, load indicator device,]
(2) Those owned by peace officers who are residents of the City of Chicago;
(3) Those owned by security personnel;
(4) Those owned by private detective agencies licensed under Chapter 111.2601 et seq., Illinois Revised Statutes;
