I fill it out as follows:
Line 1: Name of Trust Line 2: My name Line 3: USA Sign and Date. Edit: Here are some instructions from Goldman's website... http://www.guntrustlawyer.com/certification.html |

Don't think you even fill it out for a trust.
I didn't but I'm still waiting for my paperwork to be approved. |

Some examiners will kick it back, others wont. You are supposed to submit it. While the trust doesn't have citizenship it still needs to be declared from/by the Grantor.

Congrats on getting your paperwork back. I sent mine in this morning, with the form. Figured it couldn't hurt. Now the wait begins. |

Don't get your hopes too high but my check was cashed on 5-22 and my dealer received the approved form today 7-24. Not too bad at all! |
