When sending in a form 1 on paper for a trust do you need to include all pages or just the first page? The rest of the pages have no information on them.

Pages 1 & 2, printed front and back...two copies. Page 2 will indeed be blank for a trust application, but you can check the boxes if you want to.

Form 1s need to be submitted in duplicate? I've only seen that on the form 4.

Look at the top of the Form 1 on the left hand side, right below the bold, black line. It says "Submit in duplicate"

You're right...didn't see that. Only done paper form 4 and the dup;irate part is in bold in the top right.

Yea, you would think the form 1 and 4 would be a little more consistent.

And if you submit on two separate sheets per form (front and back), you'll get a rejection letter...
