It has to be a original COPY of your license with a original signature in blue ink.

So what makes the 03's different from the 01's? The letter from ATF doesn't seem to differentiate.

I believe the difference is that the ATF said it is the receivers responsibility to verify if the FFL if they accept a faxed copy and EZcheck doesn't include C&R's. If you accept a faxed copy, you have no way to verify it and you will be on the line if it is forged or has the address changed.
