Every few rounds, it will not extract the casing.  It cyles, but the empty casing is just sticking out of the chamber a little bit so obviously the next round can't load.  Is this something that I am causing or is there a malfunction in the extractor?

You have an extractor issue or chamber issue.  You are not causing it.  You probably have insufficient extractor tension, or lack of a defined hook, or just a serious build up of crud.  You may also have some serious fouling in your chamber, especially if you shot any cheap ammo that is laquer coated.  Pull your extractor and clean it if it's dirty.   Clean your chamber well if you've shot laquer coated ammo.  Past that,

Thanks for the suggestion. I will try it and see what happens.  I don't shoot the cheap laquered stuff.  Usually white box.
