Did you fit the hammer to the sear by honing/cutting the hammer hooks? Did you cut or hone the sear engagement surface and relief angles properly? These are very important, more so than the 'trigger fit'.

It's also not uncommon for the RIA to have out of spec holes which would cause in spect parts to not fit together correctly without considerable work.
