Sounds primarily like an ammo problem to me. It wouldn`t hurt to have the extractor checked. Since you indicated it was reassembled improperly in the past, sending it back to Colt for a thorough going over might not be a bad idea, although time consuming. Especially since it`s presently out of production, it would be a shame to see a collectible like that suffer needlessly or be destroyed by bad work. HTH
