I have received my form one back from the BATFE.  My question is if I am using a D.C. Industries manufacturered M-92 receiver, do I still have to engrave my name, city, and state on the receiver?

From what I understand doing my own SBR builds, is the manufacturer (the one making an NFA item) must put their info on the receiver. Their markings would only be acceptable if  the receiver was registered as an SBR by them and then transferred to you on a Form IV. Since you are making the SBR yourself and the Form 1 is under your name, you must put your Name, city and state because you are technically the "manufacturer" of an NFA weapon.
