Who makes/sells 30MOA slanted riser Pic rail?

I have a S&B PMII in 1" high 34mm rings and I'd like to mount it about 0.5" higher
with a 30MOA incline

P.S. I checked whether Larue currently sells anything with 34mm rings and they do not.
