Got this off of Subguns.com

As I posted below, i had a little mishap with some ammo this past weekend in the M16/9mm. The UZI had trouble as well, and the M11/9 just simply gave up!

The pic below is the blown barrel from the M11. The ruptured cartridge furthest left is what did it. The frame was bulged and the barrel is totaled. The frame is already straightened and is good to go again.
The next row of cartridges are what happened in the M16 with the Olympic upper. I never had trouble with this ammo before. This past weekend was a bad one for casualties.....three guns right in a row blew! The ammo is Olympic 124 grain new production.
