I had mine done with the slots at  10-2-4-7.
I heard also that the slots should be at 12-3-6-9
I like the way the 10-2-4-7 looked so I kept it there.

... Regardless. No openings should ever be at 6. I'm doing something quite similar now too to achieve an OAL of 16" (+.10). I Actually prefer to have something that "looks" like a shiny washer but if fact the hider will be permanent.

I used a crush washer, with the wrench flats verticle, the slots at 2-4-8-10, serrations at 12-3-6-9.
