Anyone ever seen a Phantom flash supressor fail on an unsupressed 5.56mm? Blossum like a Vortex?

NuCadet says his developed stress fractures and opened up like a "tulip"

Upon close inspection of mine I see that there are in fact cracks or flame eroded grooves for lack of a better term advancing from the forward edge of the slots in the flash supressor towards the front. If they continue at some point it wont be a closed end design any more.

They are about 2mm long right now. The FS has less than 8,000 rounds on it.

I would think that 8000 rounds is pretty good for a FH.  I would be happy if mine lasted that long.
