My upper has the gas tube whole drilled off-center.
It is only minor, and wouldn't bother me if I wasn't using a 4 rail handgard.
Because I am, it makes the top rail not line up with the upper's rail.

Anyone seen this before?
I will post a picture later tonight.

I thought about drilling a larger diameter hole, that is centered,
then using a bushing to hold the gas tube.

Won't it still be off center as far as the gas key on the bolt carrier?  That doesn't sound like something you want to live with, can your supplier or RRA replace it under warranty?

No. I would drill the larger hole in the correct spot.  It would not matter where the smaller hole was originally.
