I thought preban AR-15's where exempt from having the muzzle flash welded and pinned?

They are, as long as the barrel is 16" or longer, not counting the muzzle device.

is the barrel 16" or longer?  if not, it needs to be perm. secured or its a SBR.

It's an 11.5 barrel with a 5.5 flash suppressor. It still has to be welded, or pinned?

Yes, unless you go through the SBR tax stamp process.
