Okay, he tried to holster and either his finger, retention strap, or shock cord pull from his tacticool vest was inside the trigger guard. Fail!

I think it's around 50-50. You wouldn't holster a cocked and safety off single action without due care would you? Those "passive" safeties don't mean anything when something gets into the guard and deactivates the only safety. With a DA/SA you would have more effort to tell you "Hey, stop before you blast a hole in your leg!" and with a cocked and locked SA it would be a non-issue. It's because of people like the one in the vid that there is a 9lb NY trigger option for the Glock folks.
