We have:
A President of the United States
A Governor of a state
––- county commissioners
A Mayor of a City
And for the unfortunate, a Head Douche of the HOA.

Why  no (edit  elected ) chief executive for a county?

Yeah but that position is more like Speaker of the House in the fact that once a person is elected to the board the board then selects the Chair.

Some do.  County government varies quite a bit from state to state, even within a state.

Yep. We have County Commisioners here elected by district, and what used to be called a "County Executive" who is elected at large and serves four years. Several years ago the State Legislature voted to allow the title to be changed to County Mayor by Counties if they choose.
In the County where I live it's expected to be a highly contested race next year (August 2010) and a bunch of people are already making their plans to run.
