The cartridge rides up into the extractor from below and is held by the extractor as it goes into the chamber, they are shaped to work that way on many guns. If you drop a cartridge directly into the chamber you force the extractor over the cartridge rim and you can break the extractor tip on some more fragile guns, older 1911's are known to do this.
