A bubblehead buddy of mine has advised that back in the days of the Shah, the Iranian Navy purchased a submarine from Electric Boat.

He stated that he had helped train up the maiden crew, and had left the country prior to their first operational mission.  

As most bubbleheads (submariner) people know, you must make sure that all hatches are sealed prior to diving.  The Iranians, in their infinite wisdom, and hatred of insects, had installed a screen over the conning tower hatch, but allowed it to fall on the sensor that runs to the "Christmas Tree".

Yup.

1 lost sub and crew due to screen door on submarine.

My bubblehead buddy is not known for making things up, but I still am waiting for a lawsuit to be filed by the Polish Navy for copyright infringement...
