Does the body break it down, does it rot, is it ALL coughed up in time, do the cilia bring it up, or what?

It will almost always get coughed up. Lungs do not like solid or liquid invaders & will reject it; or at least attempt to.

I heard sometimes paralyzed people try to swallow food and it can go down the wrong tube and can cause an infection, they call it aspirations.

The eppiglottis (sp) spasms.  It will not permit solids or fluids to go down there.

When you drown, it spasms shut.

If you are coughing.....The bit is bounceing around on the eppiglottis.
