i found a m-11 online and says it is on a Form 4 in PA. What is this? Any help would be great.

It means that it is a transferable machine gun.  Form 1 means the person who has the gun is the original manufacturer who was not a commercial manufacturer, Form 2 means a commercial manufacturer made the gun and still has it.  Form 4 means the gun was transferred with the $200 tax paid from the original manufacturer to the current owner, with possibly a few other people in between.  If you buy the gun, it will go on a Form4 to your local Class 3 dealer and then on another Form4 from your class 3 dealer to you, each Form4 costing you $200.

Form 4 is the form used to buy full auto weapons. Must be signed by a CLEO, DA or Fed judge.

Almost correct.  Dealer to dealer transfers typically go on a Form 5, and there is no $200 tax.  The Form 5 still has to be approved by the BATF before the transfer can take place.

Acutally Form 3's are used for dealer-to-dealer transfers.

I believe a Form 5 is for a tansfer of a registered NFA item tax free, like when an item is willed to someone.

An individual (non-FFL/SOT) will possess a NFA weapon on either a F1 or F4.  F3 is used for dealer to dealer transfer. F5 is tax free transfer for repair or inheritance.

Since this gun is on a F4 in PA and you also seem to be a PA resident, there is only one form and one tax to be paid if you purchase it.  You will complete a F4 and pay one tax - unlicensed individuals in the same state can transfer NFA weapons to each other directly on a F4, no dealer is necessary.
