That is the IR laser illuminator. Think the number is AN/PAQ-4

I haven't seen this before, I assume it is offset from the front sight? How does it attach?

The PAQ-4C attaches with the same type getup that the M68 (Aimpoint) does, just a handknob on the forward rail. Some mount it on top as shown, where the beam is offset to the side of the sight post. It can also be mounted on the sides or the bottom, although I don't recall seeinf anyone stick it underneath. You do have to pay attention to the adjustments when zeroing as they will be all jacked up in different positions. I am just grateful that grenadiers in my unit don't get PAQ's.
