Now that there's a Sepsis Core Measure population, we have all sorts of buzzers and bells built into our EMR to flag Sepsis and possible Sepsis patients. Are you on an EMR?

We use Epic and have triggers on the lactate order (possibly a prompt to repeat the test within the 'defined' time?) and also have system lists (based on Problems as well as lactate orders/results) specifically to capture potential and actual Sepsis patients. Specific Quality Management, ID staff and Clinical Nurse Leaders (CNLs) are assigned to stay on top of these patients. We're not where we need to be on the measures yet, but we are definitely catching more potential Sepsis patients and that's movement in the right direction. We're in the process of building more tools in Epic for those ancillary folks (at their request) to manage the population, too.
