My guess is used error! Saw it all the time when I worked for a sporting goods store. People would think they had XYZ but really that had ABC !

Or people who hand load but don't know what they are doing!  Last are people who buy a cheap weapon that is old or not working properly and shoot it! All users error should be lucky there alive!

I seen one at Koko Head a lot time ago when the guy was shooting his AR. Big puff of smoke followed by his mag popped out in many pieces.

That's all I can remember.
