So what are M4 feed ramps? Can I modify a current barrel to have them? Do they really make a difference?

Other rifles and models may have them, they're supposed to improve feeding in full auto fire, but I'll let other more knowledgable folks jump in  This pic is of my Colt 20" upper, purchased in 1991:
