What address do I put on a Form 1 if my mailing address is different from my physical address.  The mailing address is not a PO box.

Yes, sorry.  It is for an individual.  I didn't know if it would cause problems with the addresses being different, because the SBR lower would be located at my physical address, but I would need the stamp mailed to the mailing address.

I have another question relating to this.  I used the SilencerShop Form 1 service for a SBR.  I have the mailing/physical address thing straightened out.  Now my question is for section 4h, where my name, city and state go that will be engraved.  Does the city need to be the one from my mailing address or my physical address?  I want to make sure it is right before I sign and send the form off.

If I understand your question, the city and state that are engraved is the place that you actually do the final assembly (making) of the firearm.

47 CFR 479.102
(iv) In the case of a domestically made firearm, the city and State (or recognized abbreviation thereof) where you as the manufacturer maintain your place of business, or where you, as the maker, made the firearm; and
