I haven't messed with the new barcoded forms, but I have to assume they start with the same form.

CoC forms haven't been required in years. Those questions were added to the F1/F4 in the version prior to the 41F forms.

The 16 pages sounds like you're printing the whole package, which includes three copies of the form (ATF Copy, 2nd ATF Copy which is returned to you, and the CLEO copy)...plus all the instructions. You'd only need to print that once, and don't need to print the instructions.
