I wonder if it was XM193PD or plain old XM193. He didn't say, but we've all heard about the XM193PD.

Several thoughts.

Case head failure.

Bolt carrier failure due to improper Metallurgy.

Early discharge resulting in unsupported case head.

One other thought.

The bolt is locked into the Barrel extension, either partially or completely.

Pressure was dumped directly into the upper.

Gas tube and handguards are intact.
(Failure occured before gas tap was under pressure)

Bolt carrier is obviously TOAST, and possibly indicates a pressure path back through the primer when the split upper is considered.

Which of course leads one to think about an obstructed bore.

Whatever.

The rifle failed, and did so in a manner that kept the Kid alive and just a little rattled.

Bad shit happens.

I'm glad the Kid didn't opt for a lesser rifle.

I kinda see where s-28 is going. Could a high primer be the cause? Since the bolt was locked up it seems to suggest that either the carrier key was somehow bonded to the gas tube or he had a badly dented case that allowed back pressure to escape the chamber. Just trying to learn a little today. Looks nasty though. Glad the guy was alright.

It looks like an ammo problem, it could be a bullet stuck in the barrel, as someone mentioned it was behind the gas port, the photos clearly show the gas tube is still inside the key on the bolt carrier.  Bushmaster will figure it out.
