On optics mounted M4's, do soldiers carry a handle on them. Are they issued a handle? Do they usually have iron sights in addition to the optics?

The handle is part of the M4 and M4A1 Carbines.  Comes standard as a basic issue item.

Optics are separate line items.

Unit SOP determines what's carried and by whom.

I meant to say, If they have an optic mounted on the flat-top, do they keep the handle in their pocket?

It depends on what the unit SOP is.  Some tie it off in their buttpack.  Others mount it on the side of the MWS (RAS).  Others mount the M-68 (Aimpoint) on the RAS and leave the carry handle on the upper reciever.

It also depends weather or not you have a flip up mounted. The M4A1 SOPMOD kit has enough to outfit everyone with a flip up that has an optic mounted.

There were plenty of pics that were posted here that have their carry handles attached to the sides of their RAS.

Usually thats the Marines, they are not issued a BUIS for their M-16A4's, and use the carry handle instead and often store the handle on the side of the RAS.

Not every unit in the Army has a BUIS.  They do it too.  Most common config is what I listed above in absence of a BUIS.  Even seen a soldier or two mount the carry handle on the bottom of the MWS (RAS)...obviously didn't have a foregrip.
